Diagnostic Principles and Methods

WHEN CLINICAL DATA are clear cut and match well with a selected entity, detection of the cause of a headache can be a simple matter. However the story might be obscure or in half misleading; the proof must then be weighed with care in designing more studies. The history helps most when clues given by the patient are imaginatively pursued. Alone, it often will indicate the probable diagnosis. The physical examination, as well as the neurological examination, rein¬forces the first impressions or parades other prospects to be explored. These basic principles are timeworn but sound. Guides of specific sensible price obtained from the history and examination are discussed in the paragraphs that follow. Several diagnostic axioms are included, in full awareness that generalizations are inherently dangerous, however can be useful.

Remissions in the headache history are reassuring when they are long in duration, for they usually indicate that physi¬ologic mechanisms (vascular or muscle contraction) are at fault.
